PHYSICAL EDUCATION

Prior to January 18, 2005, students participating in Independent Physical Education will be officially enrolled in the program by submitting Form #1 (pdf file) and Form #2 (pdf file) to the school office for approval.
 
In order to receive credit, students enrolled in Independent Physcial Education must submit Form #3 (pdf file) to the school office prior to the conclusion of the semester.
